It includes details about the type of information collected, its usage, protection, and disclosure.<\/p>\n \n <ul>\n <li>Patients have the right to refuse any treatment, advice, or procedure. Our doctors discuss all aspects of treatment and offer alternatives.<\/li>\n <li>For ongoing patient management, departing patients will be asked for a forwarding doctor\u2019s or Practice address.
